good question accounts will blow again and again until we learn from mistakes and failures , more over one should strict to money management lot size should depend on type of a/cs we open...
It has been same with me too. I have been wiped out 3 times during my trades. I lost all my money then. I was really stupid not to close the trade when the trends were moving against me so vigorously. But now I have recovered that loss and I am never making that mistake again.

i have lost'my'accounts minimum five times i guessd, the first one for using large lot size and it was when i had any knowledge about the lot size and MC, others for holding on the losing trades and not fixing a sl target.
i lost my trading account one time..
greed and crossing money management rules are very dangerous.. due to these factor i will lost my account.
if my greed was in money management then may be i will save my account but my greed is out of my managements so i will lost my account.

1 time before , i got a blow off of my account of 100$ that i invested in my instaforex account . after that i reduced my lot size and learned to put stop loss in my trades . after that i never get a such big blow and always try to make little profits with respect to my investments.
